Minecraft Crashes when I log into a world or a server.

      Alright, so when ever i load up a new Minecraft 1.9 world, [or log in to a server, for example hypixel.net] i immediately crashed upon world loading and got an error report. (see below)

      What I expected to happen was...:
      I would log in to my minecraft 1.9 world, and see that the new world should come up fine, but the opposite happened.
      When the world started to load the chunks, it crashed and gave me this report:

      Steps to Reproduce:
      1. Log in to minecraft account [or launch minecraft in other words.]
      2. Create new world
      3. Play newly created world
      4. Crash
