Minecraft 1.10.2

OS Version: Windows 8.1
Graphics Card: AMD Radeon HD 8400
Java version: jre1.8.0_45

Unconfirmed
If I start my client (Minecraft version 1.10.2), I can play in a world just fine. However, if I then close that world and open another, a render glitch appears.
You can see how this looks in the attached video.
Mobs are huge, blocks are teleporting in and out of sight and the game is unplayable. To fix this issue I have to close my client and restart the game.
Also attached is a crash-report, generated by holding f3 + c. (I have no clue if that is useful or not)
EDIT
Updated the crash log to vanilla 1.10.2 instead of optifine 1.10.2
