When returning from the outer end islands ( approx. 10000, 4000) with a gateway portal and pearl, I was immediately thrown into the void and killed before I loaded in. A end stone platform was still generated, but I was placed a few blocks off from it.

It was easy to recreate this bug in a creative world, just remove all the end stone nearby an end gateway on the main island, then tp out to a gateway in the outer islands. Travel in, and you will be putt off a ways from the generated mini-island.
