textures of items and blocks are swapping and mixing

      At random times, the texture of random blocks change into a broken texture(different block texture or mixture of different textures). This bug happened every time i played so long(about 1-2 hours). This also happened if there are too many entities

      I tried turning the fancy graphics on and off but it doesn't help. The pictures below are the example of the bug.
