I used a nether portal in the nether. It took longer then usual to load, when i did load i was dead and my armor, tools, etc. was nowhere to be found. 

I cant replicate this however i did find where the items ended up. The nether coordinates of the portal i used was X233 Z115. The lost items ended up in the overworld at these coordinates. 

They werent scattered around like they would if you just died, they were all at exactly this block.
