When you drop an item on the ground(I tested it with multiple items) that renders in 2d the flat side of the item faces north to south and each edge is east to west. It does not rotate. When you throw a potion entity or a bottle-enchanting it is the opposite. Its flat side renders east to west and the edges are north to south. None of these items or non item entities rotate.
