When trying to play an offline game with multiple players, when adding in second player on opening screen it shows the first players avatar even though we have made sure that each controller is a different account.  We can not exit screen and have to close application.  Not effected when only one player is playing. This has happened before and was solved when we logged into playstation network.  This time we checked that and are all logged into network.
