Every now and then my mushroom doesn't come off cool-down and the only way I've found to get it back is to die or leave the level. I've only had it happen to my mushroom so far (though I've not really been using anything other than harvester, mushroom, boots, and the gong). I don't know what causes it yet, and I do NOT have the final shout enchantment on my armor.
