There is a bug in your home camp area where you can die. If you enter either the house or church in the camp whilst rolling you can die. To simply recreate this bug roll and then press your button/key to enter the building and it will kill you. I believe this works with any door in general as we also tried it and died on a door in soggy swamp. In the clip I've provided this is what I done. (Me hitting the switch isn't what causes it.
