 Description   

When standing right against a wall, the item you hold goes into the wall. Device in picture. The item in hand is cut off or clipped inside of an adjacent wall, block, boat or other structure.

Per comments, it appears that this is most common when there is weather (rain, snow, thunderstorm, etc.).

Affects 0.15.1 alpha build 1.
For me, it only happen for slime block both in the weather/not in the weather.

Confirmed to affect 0.12.3 on iPhone 6+ (iOS 9.1). Can be reproduced consistently during weather, though some reports claim to have experienced it without.
