Looks like it's fixed with the new Nether generation. Not sure if it's fixed for worlds with the old Nether generation. |

This happens when they generate on a chunk border and the chunk where they would flow into has not generated yet. When that chunk does generate, it does not update the blocks that border it. |

Can confirm in 1.0.4.0; There are lava source blocks that do not flow unless a block update occurs right next to them. |
