This is a CraftBukkit issue.

Recommend close as this is an issue with CraftBukkit/one of the plugins, not vanilla Minecraft.

It has happened repeatedly in survival multiplayer, the farmland was hydrated, it did not decay. What's happening is not the farmland reverting to dirt, the block is instantly breaking and dropping dirt as if I broke the farmland block. It seems to happen most when left click is held down and less when it is pressed for every crop. The plugins on the server may have something to do with it; plugins the server has are Bukkit, PermissionsBukkit, BanReport, CommandBook, LogBlock, mChatSuite, WorldEdit, WorldGuard, and maybe a few more.

I can't reproduce.

If the farmland is not wet, it will turn to dirt after a random delay. So it is possible it sometimes turns immediately to dirt just after the harvest.

If you place water within 4 blocks of the crop, the farmland will be hydrated (after a while) and farmland will stay farmland.

I can't seem to reproduce this. Can you provide any other info? I hope you are playing on survival.
