Sprites often come in sprite sheets that's one image file with a bunch of sprites. The sprites get cropped to show just one sprite at a time. This one accidentally cropped the sprite sheet too far to the left, and got a bit of the fungus on a stick sprite in there, and cut a bit of the warped sign.

Basically, it was an issue with the cropping of the sprite sheet, and accidentally got another sprite (the fungus on a stick) in there.
