I've tried every combination I can think of to reproduce this issue and have not been able to.

As I mentioned previously logs are submitted automatically when the server crashes so these get investigated by Mojang.

If you're able to find a specific set of circumstances that cause a crash please feel free to log a new report.

You could be onto something with the whitelisting. I've only seen it on my server once and I believe I was whitelisting during that session too...

I'll do some testing and see if I can come up with something! Potentially good catch!

Oh yeah that makes sense, I didn't know they were being sent. I've only seen this once – if I figure out how to reproduce it I will reopen. I can't think of the exact steps except that I was using the whitelist command prior to trying to stop the server.

Is this something that happens 100% of the time or do you know of exact steps to reproduce?

In general issues such as these get closed as the crash logs are submitted to Mojang automatically and are investigated based on the crash report. Its impossible to know for sure when crashes get fixed so tracking them here is a nightmare.

If you have a way of reliably reproducing it though that can sometimes help.

World seems unaffected.
