When in any difficulty mode character will display eating animation and sounds even if there is no hunger or damage.  As well, after increasing hunger by running and jumping continuously, the hunger shanks will automatically refill regardless of the level of hunger giving the appearance that there is nothing wrong.  Once you begin to eat, the hunger bar will reduce to the appropriate level.
