Since the 1.16 nether update, the mobs can wall breach through walls and it never happen before, in 1.14. It happen when the mobs fall on a 2x2 square and get crushed by 4 blocks on each side. In most of the time, they suffocate on the blocks but sometime they going through the walls when they shouldn't.
