Resetting the nether using a third party tool is the cause of the issue. Deleting the nether does not remove the portal links because you've directly modified the game data. In order to get new portals created in the nether you have to delete the nether-side (DimId: 1) portal entries.
Use of third party tools for world editing is not supported or recommended.
I updated my world and deleted my Nether and uploaded the map to Realms. When going from the Nether to the Overworld the portals link up. When going from Overworld to Nether the portals aren't linked and it teleports me and doesn't make a new portal. It also teleported me into a lava lake.
