I've had couple of villagers disappear on me.
One might have been cured from a zombie and I am aware of this bug, but rest was bred.
The room is brightly lit to prevent any mob spawns and villagers are behind a wall so escape is also impossible.
Only thing that happened was a game crash but that was miles away from the village/breeder.
