Logged onto friend's server after first time playing in a while.  Appeared to spawn onto a monster as I took damage immediately.  A black mass of boxes appeared in my vision (viewable in third and first person) and would blink in and out depending on angle.  It moves with the character.  

Additionally, all of the creatures in the world appear only as shadows on the ground, though they still do damage.  

Both of these glitches appear on all of the accounts and worlds on our xbox, not just the server I logged onto.  None of the friends in the server had the same issue.  It seems to be localized to our xbox. 

Logging on an off, turning the xbox on and off, nothing seems to work.
