Once going into the End City and youre about to come back, throwing an Ender Pearl into the Gateway to exit sometimes does not render any blocks around you. Sometimes you can fall, but most of the time you just cannot see where you are going and might fall off into the void. One quick fix is to just save and quit (the host does).
