As soon as the world finished loading I was greeted with the "You Died" screen. I know for certain that I was in a completely safe area underground when I last saved, yet after respawning I found my items located above ground roughly 15 blocks from where I last saved. This has only happened once and I have absolutely no idea what caused it.
