Issue: After dying, a chat message by the system tells the player "respawn point was missing or obstructed" despite there being no such obstruction. The new respawn point is not the global respawn point (the initial starting squares where the player loads into the world for the first time).

Other details: player died in the nether. A ghast ball explodes on top of the player after dying. The respawn point was a bed next to a chest inside a house. This occured offline, in single player, normal difficulty.
