When I'm playing a single player world on Xbox there will be random lag spikes and the game will enter some sort of weird state. In this state I can break and place blocks but there is no breaking texture and it will revert seconds later: however it will only revet some of it. This mainly happens when I'm mining netherack with a high efficiency pickaxe witch can lead to getting stuck in walls.
