Whenever you want to go between the nether or the over world there is a chance that the game teleports you to the same coordinates that you were in in the last dimension. Ex. being in the over world at 763, 53, 431 and going to the nether portal teleports you to those same coordinates in the nether instead of teleporting you to the proper coordinates. (While typing this I realized that this was happening every time we had the bad omen effect applied to us).

It also just happened to me going to the end as well (image comparison between where you are supposed to spawn vs where I actually spawned).
