Difficulty can be adjusted in The Game Menu, even with Cheats Off, however you can't use the /difficulty command with Cheats Off. 

This isn't too big of an issue on regular worlds, however on realms, Operators don't have access to the "Game" menu, so they have to deal with commands, however, if Cheats are Off, they will be stuck in said Difficulty until the Owner of the realm comes back.
