I have 89% of all achievements finished , and yet, all of the cosmetic items I should have unlocked are not showing up as unlocked, so I messed with the settings, looking for a solution. I turned on 'Content Log History', and saw dozens of messages that all read "[Geometry][error-] Missing expected geometry file ______.json" (the underscore contains the model for whatever cosmetic it references).
