Im not sure how well you can see it, but in the image there are 3 grim guardians with quick, protection, radiance, echo, regeneration, deflect, and fast attack; along with 6 enchanted necromancers. Im not sure if the fact that 3 spawned in one room was a bug, but the bossbars above that certainly look buggy. Also i died almost instantly, even though my power level is higher than the recommended power level, and i had potion barrier and protection effects active. Difficulty was only apocalypse+6 . (sorry for the poor photo, my hands are very shaky)
