So, when using the Harvester artifact you can be launched in the air. This isn't too bad an issue except I was launched into the air off map and died in a locked part of a map (Redstone mines). I also had Final shout and Health synergy enchantments on my armor which could have played a factor of launching me across the map due to multiply explosions.

Not sure how rare a chance the super launch is but a small 2-3 block high one can be recreated.
