When a play session is done, I have followed these same steps for the last 6-9 months, ever since I started playing. I quit the game, go back into the list of games to play, delete the old version of the game, and make a copy of the game I just finished. Then I put the PS4 to sleep until I am ready to play again. At the time this happened, both the PS4 and the game were at the latest versions available. This is what happened when I lost my game: I left playing, went to the list of games, deleted the old version of the game, and made a copy of the game that I just left. When the list returned, it did not copy the game I selected, but rather a completely different game - different names, so it was easy to see. Thinking that I accidentally selected the wrong game, I deleted the new copy. And it deleted the game I just finished. Here's the thing too: when First started playing before this, I went to use my last saved copy, and it was like it had copied the wrong one last time too. I deleted the wrong one and made a copy of the right one, and started playing. And started playing normally. I imagine this would be a newly introduced bug since it never happened before in the many months I have been playing. I'm seriously bummed out... this was a random seed that I (stupidly) did not write down... and it was amazing. I was 1 map short of the trophy for getting 9 completed maps together, in survival mode with an awesome self powered railway (lots of gold used).
