1st Portal outside on the ground
2nd portal in room within mountain.

When traveling the 2nd portal to the overworld the player sees a prolonged Loading World and then inmediately dies hearing multiple damage clicks in an instant.
Making player loose all his gear as it remains nigher in the overworld nor in the nether.

Other players see the avatar in the portal while player seas "Loading".
Then chat shows message message "Player suffocated in wall".

This happens to all players every time no matter if on same network or connected over the internet.
