You spawn a mob or place an entity in one place and you go away from it over 100 blocks or so and when you come back, they turn invisible and invincible but they are still there. This happens when the area where they are turned into an unloaded chunk, especially when your render distance is at tiny. This is really annoying because you can't touch them anymore unless you reload the chunk.
