This issue occurs on both single player and SMP, but can sometimes act differently on SMP.
To repro:
1. Summon an entity:
/summon MinecartRideable ~1 ~ ~ {CustomName:MyMineCart,CustomNameVisible:1}
2. Add an objective to the scoreboard:
/scoreboard objectives add MyObjective dummy
3. Set objective's display as sidebar:
/scoreboard objectives setdisplay sidebar MyObjective
4. Set the objective's value for the entity:
/scoreboard players set @e[name=MyMineCart] MyObjective 1
-> Confirm that sidebar displays 1 entry for MyMineCart (using its UUID) with value 1
5. Kill the entity:
/kill @e[name=MyMineCart]
-> Note that the entity dies, but its score is not removed from the scoreboard.
6. Repeat step 1
7. Set the objective's value for the entity using a different value for differentiation:
/scoreboard players @e[name=MyMineCart] MyObjective 2
-> Note results at this point vary depending on whether or not you are on SMP.
Single player results:
There are now 2 entries in the sidebar with identical values (2). This will repeat almost indefinitely each time the process is repeated. After 7-10 repetitions, the sidebar becomes full and no extra entries can be observed. I assume, however, that more entries exist than are being shown.
SMP results:
Results vary depending on number of players on the server.
2 (or more?) players:
The first entry will remain in the sidebar, unaffected. No additional entry is added for the second instance of the entity that was summoned. Additionally, the score of the second instance seems to be unreachable. Though the entity exists, the score cannot be set, viewed, or tested for.
1 player:
Same as the results of the workaround for >1 player. See below.
SMP workarounds:
If all players log off of the server, then log back on, the score for the second entity can be accessed. There will now be a second entry in the sidebar, but the scores do not match.
I was able to observe "correct" behavior if I summoned the 2 entities and then kill 1 of them. There were still multiple entries in the sidebar, but the score of the second entity was accessible. I did not try this with a new objective, however. I reused an objective from seeing this issue previously and didn't think to try to reproduce the issue with a new objective.
Other details:
I have seen the issue of multiple sidebar entries equally as much between single and multiplayer. However, I have only seen the issue of not being able to access duplicate entities' scores twice on SMP (while playing with another person). The first time, I thought that the scoreboard file had become corrupt, so we deleted the file. This also resolved the situation, but probably more as a result of both of us logging off of the server.
