When selecting an item in the creative inventory, you have to wait a small amount of time for it to go into your "hand". If you quickly click than move, after that small amount of time the item you clicked does not go into your hand, the item that your mouse is currently over will be selected. (I certainly hope this is not intended)

Edit: It appears the lag is about 1 tick
