Basically, if you die after using and invisible potion, you re spawn invisible. Reproduce: 1. Drink and invisible potion. 2. die by another player. 3. click spawn after it appears on death screen. 4. you re spawn and are still invisible . This happens on vanilla servers as well as modded. Not sure if this happens from death by mobs.
