What I expected to happen was...:

I was doing a raid and I finished it but, there was still a swarm of vexes after me so I put my totem in my main hand and started running,  and I thought I would be fine and trade with villagers after they despawned

What actually happened was...:

The vexes got me, and when my totem went off my hero of the village effect disapeared

Steps to Reproduce:
1. apply a potion effect to yourself (not sure if it is certain ones I only tested slow falling and weakness afterwards)

2. take some kind of damage while there is a totem in your hand until you die
