I was flying at high velocity (~500 m/s) slightly above cloud level using an elytra and trident in the rain into unloaded chunks. When I came down slowly to land, the chunks didn't load until I was below surface level. Opened F3 menu, noticed I was at y=33, immediately panicked, and used trident to shoot back towards surface.

When the game loaded the chunks while I was flying upward and teleported my player to the surface, I died with a death message of "Player fell from a ladder." I believe this is because when I initially launched the elytra, it was from a ladder.

Death glitch could possibly either be due to unaccounted upward velocity while under surface level in unloaded chunks or due to falling at high velocity through surface level in unloaded chunks, although the latter seems unlikely given my slow descent speed. Besides elytra, armor was full netherite Protection 4 with Feather Falling 4. Possibly works as intended with delayed death until chunks were loaded, but seems to be a bug.
