When loading a cave only world and going above bedrock there are above ground lakes.

      To replicate make a cave only world.
      Go up above the top bedrock
      Look for open water pools
      Then go and break all of the stone in the water pool.

      Using this method people can easily get above the overworld roof by just breaking one block only.
