here is how you recreate it: 

you need an elytra and some rockets. set your health to 10 by the following command:

/attribute @p minecraft:generic.max_health base set 10

sometimes when you hit a wall there is no death messageand your health is full (i recommend doing this on the peaceful difficulty) and when you click respawn, you dont go to your spawnpoint, but stay at the place of your death. the version might not be correct, you can have items with attributes that give you less health even before elytras were added
