I have a server that I've been running for about a week now. It's a survival server with keepInventory enabled. However there's a user who's noted multiple times that they lose their items sometimes on death regardless. I've also experienced this. I've enabled it multiple times, and yet they still lose their items on occasion. 

The server doesn't have any plugins or datapacks installed, it's the base vanilla game currently.
