This certainly sounds like a very obscure issue, but I was messing around with some of the gamerules and effects in a survival world when I ended up setting the random tick speed to 999999999 and gave myself a nightvision effect that would last for 999999 seconds. Then, I was messing around with an elytra on in an igloo when I noticed that if I jump into a cauldron and proceed to shift jump into flying with the elytra, I will stay in the flying animation even if I take the elytra off. I did some more testing and found that the issue seems to lie with setting the randomTickSpeed at such a high value because I couldn't replicate the bug with just a long nightvision or with no additional changes. Additionally just having a random tick speed at that level was enough for the bug to occur. In all cases, I would try to save and quit the game and it would sit on the screen where it says its saving the world and nothing else would happen.
Like I said, though, it seems like a very obscure situation and I'm not sure what about the tick speed is making this happen.
