This is a serious issue that has been ongoing for several versions now. Many of the larger Java servers can no longer support their playerbases due to the entitytick lag causing severe lag on servers.
Forreference - I have been running my Java server close to a decade now (it's 10th anniversary is this July), and this was never as big of an issue as it is now. It's only gotten progressively worse with every new mob addition to the game. For example the new Pillager and wandering trader mobs take vastly more resources than any other mob in the game previously and we had to disable these mobs in our world just so that the server could run at a consistent 15-20tps. But even that drops below and into the single digits if we ever have more than 15 players online at one time.
Here is a timings report with only 2 players online. There are no mob grinders running, or anything out of the ordinary. This is simply vanilla survival on an Intel i7-7700K @ 4.5Ghz

I am not a developer or have any advanced knowledge of the sort, but I have been told many times that this issue is caused primarily because mob AI processing is handled on a single thread of the processor, and is not optimized efficiently.
