Items in a fox's mouth will occasionally disappear.   It seems to happen consistently on world load, but also seems to happen randomly.  It is possible the item is being dropped for food, as the issue was observed in various fox-operated farms, but the dropped item is not collected by hoppers and appears to vanish completely.
