I have a hopper clock (using sticky pistons) that powers 4 other sticky pistons in intervals. One of the 4 sticky pistons getting powered by the clock, as well as one sticky piston on the clock lost its base and as a result stopped working.
In both cases the pistons were extended and when the extended part gets broken a invisible block, in a sense, remains where the base was and no blocks can be placed there anymore cause redstone systems to stop working.

1.png - This shows where the first piston was, next to the smooth stone block (The extended part was broken)
2.png - This shows the sticky piston on the clock where the extended part is not broken but the base is missing
