This bug seems to start happening after a large radius (about 5K blocks) of land is explored and and the player has places and broken a lot of blocks. I started noticing this after I've played the same single player survival game for 1.14 days and appears to be getting worse the more blocks I place/break. I have occasionally seen "block lag" occur along with this bug but I can't tell if this is caused by block lag or if this issue causes block lag.

As an additional note, I've given minecraft 16 GBs of RAM to use and didn't seem to have an affect on it.
