It has happened with 2 swords and a pickax. Once while attaching silverfish, once and wither skeleton, and once a zombie. Right after killing a mob (except the silverfish not sure it it died, too many of them) the tool I was holding disappeared (they did not break, they were all diamond tools and had will over half of the durability left)
