If standing too close to a gate and you open it, player is trapped, unable to move or rotate. If trapped but can access gate hitbox, breaking the gate releases player.

Reverse placed doors act in same manner (don't know if regular placed doors act this way).

Logging out, then in does not release player.

Unless player is able to break door/gate, only way to release is via /kill or to be killed by mob.
