How to reproduce: Make a ceiling (say 5x5) and put an item frame on the ceiling in the middle.  Place an item inside.  Pop the item out.  It does not go back into your inventory.  Instead, it floats on top of the ceiling.

Probably this is due to the item "popping up" which makes it phase above the block?
