This is a Pre-release 6 bug, (It wasn't in 5) so the off-hand doesn't work when you have nothing in your main hand. I try to place say a torch and it places, but it disappears. I tried it in both survival and creative and it did the same thing, but when you put something in your main hand like a sword, it works.
