This bug is found in the Multiplayer section of Minecraft (Official 1.12 Server). It happens when you try to quickly power (a) piston(s) in a repetitive way, or making something like a piston elevator.
This bug cannot be fixed by adding more ram to the server or the client. Instead, makes the effect worse. Changing gamerule "randomTickSpeed" does not help either.
How to Reproduce
This bug was discovered when I attempted to make a piston elevator, but it was also found to also work in a piston clock (Picture above/below). The piston randomly moves, and when used in something like an entity elevator, will glitch the entity off the piston block (and even if player is standing on a block being affected by the piston)
Important Note
A similar report was found on this bug (MC-116122), but this issue exists on the 1.12 update, whereas the previous issue was both resolved and reported on the 1.11.2 update. Also, the design has been modified by Youtuber Mumbo Jumbo, and this new design was uploaded less than a week ago.
For reference: The 'piston elevator' mentioned in this bug refers to the piston elevator in this video (http://thatmumbojumbo.com/video/nOVlXEw8Sa8), with the only difference that the build was made on a multiplayer server.
