I was playing around with the totem of undying to see what activates it. When I gave myself the effect "minecraft:wither 1000000 255" and died of it whilst holding the totem, it didn´t disappear. I gave myself the wither effect again and suddenly the game crashed.
I tried it again multiple times and there the game crashed immediately upon death. The amplifier of the effect doesn´t matter but it has to have a duration of 1000000 (infinitely).

To recreate the issue you have to:
1. be in survival mode
2. hold a totem of undying
3. give yourself the wither effect with a duration of 1000000
4. wait until you die
